Deploy a model into a team with no authority and watch decisions stall.

AI does not fix slow approvals or unclear ownership. It exposes them. When you drop a model into a team with no escalation path, the slowest human in the chain becomes your system clock.

Detectors create events. Events demand decisions. If no one owns the decision, you don’t have intelligence. You have a backlog with branding.

Here’s what actually happens.

Last spring, a consumer marketplace rolled out an account takeover model. It was good enough—flagged logins with suspicious patterns and pushed alerts into the trust-and-safety queue. The analysts could investigate, but they could not lock accounts without a manager, and managers needed compliance on anything affecting payouts.

By week two, the queue tripled. Analysts begged for faster approval. Managers asked for “more confidence.” Compliance asked for an SOP. Product said they’d “circle back” after launch week. Meanwhile, customers kept getting drained and support waited for guidance they never got. The model was blamed. It wasn’t the model.

Under pressure, the team tried to tune thresholds. Precision edged up, recall dropped, incidents kept leaking. Slack filled with “anyone able to approve?” messages. On Friday nights, no one answered. By month’s end, leadership quietly turned alerting to “monitor only” to “reduce noise.” That’s not risk management. That’s surrender.

This is the default outcome when signals have no authority. Detection without a decision path is a cost center disguised as progress. You pay twice: once to build and run the model, and again in delay, loss, and trust erosion.

The uncomfortable trade-off: either grant front-line authority with a false-positive budget, or keep multi-step approvals and accept slow, expensive decisions. There is no third option where models are perfect and no one takes risk. If you want speed, someone will lock a good account now and then. If you want certainty, you will miss windows and eat loss.

Average teams pretend there’s a tuning trick that dodges this. They optimize ROC curves. They write Jira tickets for “add context.” They create triage channels with eight people and no owner. They “pilot” for quarters. They run dashboards and call it governance. Nothing actually changes in production.

Strong teams do something else.

They name a single decision owner per signal class. Not a committee. A person on an on-call rotation with the right to act.

They map a simple escalation tree with time limits. Example: model fires, analyst has 10 minutes to act within budget; if exceeded or ambiguous, it auto-escalates to the duty manager; if still unresolved at 30 minutes, it pages the exec on duty. The page isn’t for awareness. It’s for a binding decision.

They set thresholds and budgets in advance. “We will accept X false locks per 10,000 accounts per week to cap takeover risk at Y.” They don’t negotiate this mid-incident. They log every action and review drift weekly. They change thresholds after a postmortem, not during a fire.

They instrument time-to-decision like a core SLO. Not just alert volume or model precision. If a path exceeds the SLO, they fix the path before they touch the model.

They give the runbook teeth. Actions are wired: block, hold, route, notify. Actions require no extra approvals inside budget. Out-of-budget requires an explicit override by a named role, recorded and reviewed.

They rehearse. Once a month they run game days: flip a feature flag, trigger a burst, force three escalations, and see if the line holds. If it cracks, they adjust the path, not the paint on the dashboard.

What changes in the next 12 months and why?

First, buyers will stop funding “detectors” that can’t answer a blunt question: what can this model stop, start, or change without me? If your answer is “create a ticket,” your project is at risk. CFOs will cut it because it has no lever.

Second, a new operational role will harden: the on-call decision owner. Not a product manager. Not a committee chair. An operator with authority tokens mapped to budgets and thresholds. Compensation will shift to pay for this responsibility and the rotations that come with it. It’s cheaper than slow loss.

Third, vendors will pivot from model features to authority features. Audit trails that bind detection to decision. Native escalation trees. Time-to-decision metrics. If your internal stack can’t show who decided what, when, under which budget, auditors and customers will start walking.

Fourth, alert pipelines that don’t produce actions will get throttled or shut off. Data and ML leaders will redirect headcount to decision engineering—wiring actions, budgets, and runbooks—instead of pushing another percentage point of precision that doesn’t move a lever.

Finally, executives will get less tolerant of “monitor only.” If your model can’t take a bite out of a metric without a meeting, it isn’t an operational asset. It’s research. That’s fine—call it research and fund it like that. Just stop pretending it’s a control.

If you are about to ship a model, do this before launch:

  • Write the decision. Exactly which action the model is allowed to trigger.
  • Name the owner. A human, not a team. Publish the on-call calendar.
  • Set the budgets. False-positive caps, loss limits, time-to-decision SLOs.
  • Wire the actions. No copy-paste into Slack. Buttons that change state.
  • Log everything. Decisions, overrides, missed SLOs.
  • Drill. Break glass once and see if it holds.

You will be tempted to dodge the trade-off. You’ll try another feature, another threshold, another note in the SOP. That is delay theater. You are either going to hand someone the keys or you are going to keep paying for noise.

Pick a side: will you give your model a named owner with the right to act inside a written budget, or will you keep buying detectors and pretend a queue is a decision?
